After the Challenger space shuttle disintegrated before the eyes of America in a ball of smoke in 1986, killing seven, investigators discovered emails discussing concerns about the seals on the rocket boosters.
Yet, those emails never got to the right people or sounded the alarm loudly enough about the dire consequences of the seals’ possible failure. Thus, one of the greatest disasters in the history of the American space program occurred.
